Go to the MTNA site and fill out the application form.

Dues for MTNA are indicated on their web site. For NHMTA, it's $15. The individual membership year begins July 1 and ends June 30. Student membership begins October 1 and ends September 30 and has a reduced membership rate. Again, refer to MTNA for theirs. NHMTA is $7.
